Release of Information

The Health Information Management department is committed to protecting the privacy and security of your health information.

You may gain access to your medical record by stopping by or contacting the Health Information Management Department in the Administrative Building for all medical records.

Please be prepared to fill out an "Authorization for Release of Information"
or "Patient Right of Access Form" to gain access.
